The Assessment Guidance course - available by accessing the 'My Courses' page on OCA Learn - provides a document titled 'Selection of Learning Log Entries - Assessment Submission Template' in the Selecting Work for Assessment section.
First, access this document link and it will open a 'Copy document' page, upon which you can select 'Make a copy'; this will save a copy of the template to your own GDrive - drive.google.com - so you can edit it.
Once you make a copy, the document will open automatically.
You will need to insert your learning log links into this document and provide any relevant context. Scrolling down the document, you will see that the Assessment Team have created an example of how to complete the table.

Once you have inserted all the links to your learning log that you wish to submit, you will need to download a copy of this document to your device. To do so, select 'File', 'Download' and 'Microsoft Word'. Selecting Microsoft Word will ensure that your assessor will be able to open the document with Google Docs as they are cross-compatible.
Now you have a copy saved to your device, you will need to access your Assessment submission folder - this should have been shared with you by the Assessment Team via email - and upload it to the relevant folder. See the How do I submit for Assessment via GDrive? article for more information.
